Amendment To 'Fit Note' Requirements

From 10th Dec 2021, the Government have amended the length of time that Employees can take time off work if they're ill. You now need to provide proof if you have been off ill for more than 28 consecutive days. See the 'Fit Note' tab under 'Services' for more information.

B12 Injections

Covid Passports / Certificate

Patients who are registered with a GP in Wales are now able to obtain their vaccination status via the digital NHS Covid Pass, after they have verified their identity using the NHS log-in. If your immunisation history is missing please telephone your local health board, the number for this area is 03000 840004

Covid Vaccination Programme 

We have now finished administering the COVID-19 vaccination here at Plas Y Bryn. If you were unable to attend for your 2nd jab, or you now wish to receive the vaccine, please contact the booking centre on 03000 840004 to arrange an appointment. 

Training Afternoon  

The Surgery will be closed for training between 1.30pm - 5.30pm on Thursday February 10th 2022.
